Application Packet - New Students

Please follow the steps below to complete the application packet:

Application Packet

    1. Application Form - Complete the Application Form
    2. Resume - Submit a 1-2 page resume.
    3. Recommendation Form - Provide two letters of recommendation from individuals who are knowledgeable of your professional accomplishments, community service, or academic record. 
    4. Endorsement Form - The form should be completed and signed by your Library Administrator, or if applicable your academic advisor or a faculty member who is knowledgeable of your academic record.  
    5. Personal Statement Instructions
    6. Student Service Requirements -  Print and sign this document for inclusion in application packet
    7. Application Checklist and Submission Instructions
